ELI5: is the questionable judgement of a few individuals around the world the only thing stopping a nuclear war? if the leader of a country with nukes wants to hit the red button tomorrow, is the only thing stopping him the fact that he doesnt want to? seems like a lot of power to give a few leaders

guys like kim jung un or even obama. idk, obama goes crazy in the middle of the night and tomorrow morning decides he wants to nuke moscow for whatever reason. could he just... do that? boom, start WW3? is there no vote or something between a few, top military personnel or is it literally just his decision? same goes for other world leaders of countries with nukes.
